Payment for Services
Payments for examinations,
cleanings, x-rays and other routine services are due at the time services are rendered.
We accept Visa and Mastercard for your convenience.
Insurance Policy/Pre-determination for Services
We understand your confusion and frustration with insurance requirements, restrictions and paperwork. As a convenience to you, our office will submit charges to your insurance carrier. However, please remember that insurance is considered a method of reimbursing the patient for fees paid to the doctor and is NOT a substitute for payment.
You should be aware that each insurance company "accepts" a different fee that they consider "usual and customary". These fees are set together by your employer and insurance company and may not reflect the normal fee charged for a particular service in this or any other office. We have no control over the contract negotiated between your employer and insurance carrier and we do not adjust our fees to meet the needs of your insurance company.
You are responsible for any fees that your insurance company does not cover. If extensive periodontal treatment is necessary, you will be given an estimate for the proposed treatment plan, however, specific financial arrangements must be made prior to initiating treatment. We are sensitive to your financial circumstances and we encourage you to be open with us so that misunderstandings can be avoided. Outside financing is available for those who qualify.

Infection Control
As dedicated professionals, we are deeply concerned about protecting our patients from infectious diseases. Our infection control program goes beyond the guidelines mandated by the Occupational Safety and Health Administration (OSHA). Because of the special trust you demonstrate in allowing us to provide you treatment, we are committed to providing you with the maximum protection and safety.

Emergency Care
If you have a problem requiring emergency treatment, please call the office so that we may see you immediately. After office hours, call the office for a recorded message with phone numbers to reach Dr. Ezis or Dr. Blume. One of us will return your call as soon as possible.

Appointments
All patients are
seen by appointment. When calling to schedule an appointment, please let us know what
times are more convenient for you. We will always do our best to accommodate you within
our appointment hours.

Cancellations
If you cannot keep an appointment, please notify the office. We reserve the right to charge for a missed or cancelled appointment without 24 hour notice. Our office does not "double book" and your time is especially reserved for you. Most appointments are long and without advance notice, we cannot fill your appointment. Once you have made an appointment, remembering and keeping it is your responsibility. Our office will confirm your appointment, so be sure to allow us to keep your records up to date with your current phone number(s).

Telephone
We encourage you to call with questions you may have concerning your periodontal condition. Our staff has been well trained to answer your questions or relay answers to you. If they are unable to answer your questions, Dr. Ezis or Dr. Blume will call you at their earliest opportunity.
